Mystery hinge issue

I’ve read almost all the threads on here about loose door hinges. I’m trying to get As much input as possible before tackling mine. My situation is a little different from what I’ve read. and I may being too gentle. The lower screw on our upper hinge appears to be loose; when we open the door it pulls away from the frame a couple millimeters. I thought it would need simple tightening but when I pulled the drill/screwdriver out the bolt was already really tight. I didn’t go at it too hard (a 9 out of 15 on my Dewalt) because I was nervous to break it off. The hinge appears to lay flat when door is closed. And when opening the bolt remains in the same place within the hinge, pulling out and going back in with the hinge. Has anyone seen or heard of this? I’d assume the nut had come off inside if the bolt was spinning freely. Any thoughts? Other things to check?
