Is there any way other than...

I have a bolt head (?) coming up from the floor and it is tearing the floor cover.

At first blush I thought it was probably a bolt that needed tightening and looked under the trailer only to find I had to drill out a bunch of rivets in the bottom of my 22 foot 2004 Safari to even look and see if there was a nut on the bottom to tighten. from the placement, I figured it was probably a bolt that held the floor to the frame, but since I can't see it I don't know what the easiest fix is.

Anybody know what is happening here and if there is an easy way to fix this?

On my unit there are no floor bolts, merely large self tapping screws that hold the plywood down. I would suggest using an Exacto blade to finish cutting the semicircle a little further around the head so you can access the top. The screw uses a large Phillips head on my unit which I suspect is the same on your unit. You can glue the floor covering back down on top of the screw head once retightened. You should acquire some scratch repair crayons to fill the small crack around the screw head and camouflage the repair spot.
