2 of every picture

I'm still getting 2 pictures posted when I try to upload 1 picture.

Here is what I am doing:
1. I upload picture from my file on the first screen.
2. The second screen comes up and I type in all the details and leave the check box checked next to the picture then select the finish (can't remember the name of the button) button.

How I've tried to correct it:
1. I've left the box unchecked on the second screen and nothing uploads.

Please help me post pectures so that only one uploads. I have a very slow connection speed so it takes a long time to edit all the duplicates out.
