I just checked my Equal-I-Zer hitch, the welding does not look like yours, I doubt they would supply a shank like that.
Yes, I am totally convinced that the original hitch installer substituted the original OEM shaft for a longer one, and selected a generic shaft from some unknown or unknowable manufacturer of generic parts. I don't blame Equalizer for this failure.

Hummm; Equlizer, Andersen, PP, and HaHa have problems and the list goes on. Is there a hitch that doesn't have a problem? What is causing the problems; the TV, TT, hitch weight/distribution, or anti-sway? Are we trying to make each of these components do more that what they are intended for? Just thinking cause and effect.

Brake safety cable

Hello, I'm real late to this thread, and did NOT read every post. But that picture is SCARY. And, in case no one else pointed this out, the safety brake connecting cable is run in a way that I doubt it would pull the pin if the trailer and TW separated.

Glad it all worked out.
