shower sealing - screws?

Shower screws.....how do they seal? It's a screw with a plastic cap. Is there supposed to be sealant under the screw? How do they do it? What have you done to insure no leaks? Seems like a dab of bathtub caulk would be appropriate. Appreciate your input.
