I looked everywhere for one of these faucets when mine went bad in 1995. At that time, before so many things became available on the Internet, I couldn't locate anything even vaguely similar. When I was sourcing materials for the refurbishment project on my 1920s bungalow, I found a very similar faucet design with the needed diverter that you can find at this link.

I purchased most of the faucet sets for my refurbishment from the company above, and have been very pleased with their quality.

My bath faucet on our '65 died a few years back, and it ended up being a learning experience. The 3.5" faucet is what is used on clawfoot tubs- you can sometimes find one in a dusty cardboard box in the oldest hardware store near you. Ask the oldest employee you can find. Of course, a search for "clawfoot tub faucet" on ebay will get a huge amount of overly ornate weirdness, and a few very useful options.
You may also be able to bring the faucet in to the hardware store and just replace the worn out parts.

In a 1964 GT that I had to refurbish, the owner had me to elongate one of the holes in the fiberglass and we installed a standard 4 inch spread faucet. The enlarged hole is hidden behind the faucet trim plate.
