Intermatic may have something in their WP1100 / 1200 series that fits. Plastic Covers | Intermatic, Inc.

else, Connecticut Electric has this CESMPS13HR, though the box Minno recommends looks deeper inside the cover?

add 4 new tapcon screws, and the matching-size masonry bit!
