They come with 6 contacts on the back, (: : like this. Connect power and ground to the middle contacts. Cross connect the outside contacts, i.e. right front to left rear, left front to right rear. Connect the power and ground wires from the fan to one end only, i.e. right front and right rear. The switch will connect straight in one direction and reverse in the other.

Disregard my last 2 posts. The motor is not reversible. I bought the switch and installed it and much to my chagrin, reversing the polarity also reverses the polarity of the field coil magnets and the fan still goes the same direction.
